Yes, depending on the type of form. All forms do require an appointment If you are looking for a treatment form, our Walk-In Doctor or your Family Doctor will be able to do this. For WCB, Insurance and Disability Claim forms, you MUST book with your family doctor. Depending on the form there may be a fee that varies between Doctors.
